CHILD HEALTH AND DISABILITY PREVENTION PROGRAM (CHDP)
 
     
Purpose
 
  • Assure that eligible children and youth have access to on-going healthcare
 
     
Who is Eligible?
 
  • Medi-Cal beneficiaries from birth to 21 years
  • Non-Medi-Cal eligible (less than 200% of the Federal Poverty Level): from birth to 21 years
  • Any child enrolled in Head Start or State preschool programs
 
     
  Who Provides the service?  
 
  • CHDP Providers including:
    • Private Physicians
    • Primary Care Centers
    • Clinics
    • Public and Private Agencies
 
     
  Where are the services provided?  
 
  • CHDP Provider offices
 
     
  What is offered?  
 
  • Free physical exams to eligible children
  • Coordination of follow-up care
